Payment
Please refer to the "Policies and Procedures" form for current rates. Fees are due at the time of service. I accept cash, check, or credit card as payment.

Cancellation Policy
I am happy to cancel or reschedule your appointment if necessary. Appointment times are reserved just for you. All cancellations must be made at least 24 hours (1 business day) in advance of your scheduled appointment otherwise you will be charged the current rate for that appointment. You will also be charged if you fail to show up for your appointment.  Emergencies will be taken into consideration.

Insurance
I do not accept insurance as a form of payment.  You may contact your insurance company to inquire about out-of-network benefits and what the reimbursement may be for my services. I can provide you with a billing statement which you can submit to your insurance carrier, but reimbursement is not guaranteed.
